v0.4.0

Bug fixes (trace 019e8a21):

  • compaction rebuild no longer emits orphaned tool-result messages
  • errored turns now persist their partial history (fixes last_seq=-1)

Includes the Plan B pack safe-defaults BREAKING changes already on main (sub-agent narrowing default, unknown sub-pack id hard error, new inherit modes, write-tool path constraints).
